| Leo's Journey with Project Permanence |

Leo presented with a history of depressive and ADHD symptoms. The client was being reunified with his biological mother along with his 7 siblings. There were many needs for stabilizing the placement. The Project Permanence (PP) team came in to help find potential housing options, and coordinate with the client's other service providers to get him the appropriate services in school. PP was also able to provide community resources for summer activities for Leo and his siblings. PP staff were able to assist in finding new housing options when their current house was in danger of foreclosure due to the landlord's financial troubles. The caregiver wanted to stay in the same are if possible to avoid uprooting her kids from school and the safety of their current neighborhood. The Family Advocate on the team was able to work closely with the caregiver to locate other possible housing options and access lists of affordable options from nearby areas as well. Luckily a new owner purchased the property and was happy to let the caregiver and her family stay. The Family Advocate was also able locate resources for clothing and transportation assistance. After initial resistance to services the caregiver was able form a strong relationship with the Family Advocate and was very grateful of the support. The Family Advocate on the case provided information about parental rights regarding the education of her children and used personal experience to help the caregiver navigate the IEP process. The caregiver and PP team worked together to address structure in the home and create a network of support for the caregiver's own journey to maintain sobriety and receive assistance with her mental health needs. Leo had an IEP when Project Permanence came onto the case. Project Permanence staff were able to coordinate with the school and other service providers to attend IEP meetings and assure Leo was given the necessary support. PP staff worked with Leo's caregiver to go through the IEP and understand how to advocate for her children through the IEP process. Leo was behind in many subjects due in part to his ADHD symptoms and other behavior problems. The Community Liaison worked with Leo and other service providers to identify and practice appropriate copings skills to increase Leo's ability to focus during school. PP staff worked with the client, his CASA worker, and the resource teacher at the school to determine the client's academic strengths and weaknesses so that the client's schoolwork was at the appropriate level. The client was able to make strides in his ability to communicate his feelings. This was accomplished through the combined efforts of PP staff along with the caregiver and the client's individual clinician. PP were able to assist the client in gaining vocabulary to express his feelings more accurately. PP staff also processed with the client about communication breakdowns in order to help the client reflect on those difficulties and find other ways to communicate in the future. |
